A 14-year-old boy is brought to the clinic by his parents for a 6-month history of intermittent, crampy abdominal pain, non-bloody diarrhea (4-6 loose stools daily), and a 5 kg weight loss. He reports profound fatigue, decreased appetite, and has fallen from the 50th to the 10th percentile for weight. He has no history of foreign travel or sick contacts. On examination, he is afebrile, appears thin, and has mild tenderness in the right lower quadrant without guarding or rebound. There is no perianal disease. Initial labs reveal a hemoglobin of 10.2 g/dL (microcytic), an ESR of 48 mm/hr, a serum albumin of 2.8 g/dL, and a negative stool test for infectious pathogens.
