A full-term newborn male is delivered via spontaneous vaginal delivery. The pregnancy was complicated by polyhydramnios. Immediately after birth, the infant is noted to have severe respiratory distress, with tachypnea, retractions, and cyanosis. On physical exam, the abdomen appears scaphoid. Breath sounds are markedly diminished on the left side, and bowel sounds are audible over the left hemithorax. Heart sounds are best heard over the right chest. Preductal pulse oximetry shows an oxygen saturation of 85% on room air.
