Clinical Vignette Assessment
A healthy 18-month-old boy is brought to the Emergency Department after pulling a cup of hot tea off a table, spilling it onto his chest and right arm. He cried immediately and has been consolable by his parents. On primary survey, his airway is patent, breathing is unlabored, and he has strong peripheral pulses. Examination of the burn reveals confluent erythema and multiple large, fluid-filled blisters over his anterior chest, right shoulder, and the anterior aspect of his right arm. The blistered areas are very painful to light touch. The parents estimate the spilled liquid was about 180°F (82°C). His weight is 12 kg.
