A 7-year-old boy is brought to the Emergency Department 30 minutes after falling onto the handlebars of his bicycle. He reports severe, constant upper abdominal pain. On exam, he is alert but anxious. His heart rate is 115 bpm, blood pressure is 105/70 mmHg, and respiratory rate is 22/min. He has a 3 cm area of ecchymosis and tenderness in the epigastrium. His abdomen is soft but tender to deep palpation. A Focused Assessment with Sonography for Trauma (FAST) exam is negative for free fluid.
